Covers annual visit, unlimited phone access, prescriptions, prior authorizations, letters, home visit. Also covers urine drug screenings, collaboration with therapists and up to weekly visits during stabilization and high stress times.

Travel fee included within 5 miles of office address. 

Greater than 5 miles but less than 30 miles $30 travel fee per visit

Greater than 30 miles $0.60/mile over 30 miles.

Need to understand something that makes no sense? Providers threw things at you and your head is spinning? Will provide up to 60 minutes and/or original written materials to help you understand and reinforce what you need to know to make health care decisions for yourself. 30 minutes session included in membership fee, more extensive education may require additional fee for members.

Price depends upon length of consultation. Included in membership fee.

Will review your existing records, diagnostics and laboratory results and provide written summation of evaluation. Included in membership fee.

Walking you through the myriad of decisions related to end of life care including completing Physician Orders for Life Sustsining Treatment (POLST) forms and other advanced directives. Included in membership fee. 

When you need to meet Healthcare on its terms but you do not want to do this alone.  I will accompany you, ask questions for you, explain what is going on so you can understand and ensure that your choices and decisions are heard. Per visit pricing negotiable for long visit access to healthcare such as inpatient hospitalizations.

Standard doctor’s excuses for work and school, prescriptions and refills, ordering and evaluating diagnostic and laboratory tests, maintaining medical records, vaccine records, and other standard services necessary to provide continuous health care services do not incur any charges.
